As an assessor, Central Adelaide Aged Care Assessment Team (ACAT) you will undertake comprehensive assessments of aged clients in a variety of community and hospital settings. Your primary objective will be to undertake comprehensive needs based assessments of clients using the National Screening and Assessment Form (NSAF) within the myAssessor application and the My Aged Care client record. You will enter accurate data from assessment information using the myAssessor application and My Aged Care system.
The assessor is responsible for providing up to date information on appropriate aged care pathways that the client may be eligible to access. These include, but are not limited to community, permanent residential, residential respite and flexible care options and both State and Commonwealth funded pathways available under the Aged Care Act 1997. As an assessor, you will liaise with clients, carers, other health professionals, service providers and other internal and external stakeholders.
