The PCLI Coordinator will work with the Operations Manager to provide management support to the Kalparrin team and service where a number of long length of stay consumers currently reside.
This role provides an opportunity for a highly motivated and skilled mental health professional to lead the implementation of the Pathways to Community Living Initiative (PCLI) in the Illawarra Shoalhaven Local Health District. The PCLI is a State wide Ministry of Health initiative to develop models of care and work to ensure people with enduring and complex mental illness who have been consumers in an inpatient setting for over 365 days move into an appropriate supported accommodation setting in the community.
The PCLI Coordinator will work closely with all teams and services within the ISLHD Mental Health Service to implement an assessment, planning and support process to identify the support needs of consumers, identify appropriate accommodation options and support services to meet those needs to facilitate the transition of the consumer from an inpatient setting to a community accommodation option and specialist support services in line with the broader state-wide Mental Health PCLI being coordinated and managed by the Ministry of Health through MHDAO.
