Clinicians have asked NSW’s ombudsmen to investigate claims of systemic failures at NSW Health.

Five former staff members at Broken Hill Hospital last year blew the whistle about mistakes that led to the death of local teenager Alex Braes.

The same group has now written to the New South Wales Ombudsman to ask for an investigation.

“Our complaint concerns systemic failures within [the Far West Local Health District and the Ministry of Health] which we believe have led to avoidable deaths of hospital patients,” the clinicians wrote to the ombudsman.

They said appeals to Health Minister Brad Hazzard and departmental secretary Elizabeth Koff had not been effective.

The ombudsman's office has confirmed it is considering the complaint.
